Was Ephesians written to Ephesus? - Ephesians 1:1 - Textual Criticism

How sure are we that the book of Ephesians was written to the Church in Ephesus? The letter begins "Paul, an apostle of Christ Jesus by the will of God, to the saints who are... in Ephesus," so shouldn't that settle the matter? It might, but many of our earliest manuscripts don't include the phrase "in Ephesus."
